STEP 3. Fill out the forms completely.

STEP 4. Scan the forms and email them back to me at Marty@MartySimpsonMFT.com PRIOR to your first session with me. I cannot begin Psychotherapy with you until I am in receipt of the forms.

STEP 5. Once emailed, mail all the completed, signed forms to me at the address on the forms, so I have the originals for your chart. I cannot continue therapy with you without the original forms/signatures.

If you are unable to print the forms I can mail them to you, however, this may delay the start of our work by several weeks. Please let me know by text if this is the case.
